Preparation for Facilitated Case Conferencing

Careful preparation is important to ensure a case conference is effective and is seen as valuable to family, aged care staff and external providers. It is important to ensure the case conference is at a time which meets the needs of participants, prioritizing those whose input is most needed and accommodating to those with challenging schedules (e.g., GPs). To ensure the meeting is effective, relevant information needs to be gathered and (where necessary) circulated prior to the meeting.
