It is important to commence preparation for a case conference well in advance.
- Decide who should attend based on the topics for discussion and need for decision making, and check staff rosters
- Contact the family to discuss who should represent them and arrange a suitable time
- Where a GP is needed, ask practice staff to include the case conference in the GP’s schedule (this may be in the form of an appointment in the GP’s diary, and call in by teleconference)
- Complete the planning checklist (179kb pdf) to ensure you are prepared in advance.
