Functional or Clinical Status Decline

Any change in a person’s level of functioning can be a sign that dementia is getting worse and their care needs may be changing.

Changes may include:

  • Increased frequency of falls
  • Reduced activity/interaction
  • Inability to be out of bed
  • Reduced awareness of environment

Assessing the person's changing function and communicating this to the family and care staff can ensure all members of the team work together to keep the person safe.

Further information to assist in the preparation of a facilitated case conference, suggestions on who to include and tools to assist in conducting a case conference can be found using the menu.
