What is Palliative Care Connect?

Palliative Care Connect is a statewide service that provides information and links to palliative care and bereavement supports in South Australia. It provides an information line for people with life-limiting families, their families, and health professionals as well as access to the Palliative Care Navigators service who serve to link people to the appropriate supports or services.

CareSearch has partnered with the South Australian Department for Health and Wellbeing (SA Health) to develop and pilot an online platform for connecting South Australians to services, resources and supports that could help them navigate their palliative care journey as a patient, carer, or family member. The platform will enable the user to:

• find health and care services based on postcode or suburb using a searchable directory
• link to common support pathways, and
• initiate a request for a phone call with a Navigator from the Statewide Palliative Care Service.

The platform is anticipated to be available in early 2024.
