Planning dementia care through case conferencing
Guidance on this website was informed by the IDEAL Project, which was funded by the Department of Health and Ageing from 2012 to 2015.
The investigators would like to acknowledge the contributions made to content on this website by the managers, staff, residents, families and Palliative Care Planning Coordinators (PCPCs) at each participating aged care facility; the health professionals and consumers who helped train the PCPCs; the teams in Sydney who ran the project and collected data; and Adele Kelly (BSc (Hons), from HammondCare who collated the content and designed the website with support from Ruth Murton at CareSearch.
